📁 Requesting Your Medical Records
In compliance with California law, we will retain all patient records for at least seven (7) years from your last date of care.
To request a copy of your medical records, please send an email to:

Your email should include:
- Full Name
- Date of Birth
- Your preferred email address for receiving the records
- *Picture, Screenshot, or PDF of your confirmation/receipt of payment from stripe for the records ($25)

🔐 How You’ll Receive Your Records
Once your request is processed, we will upload your records to a secure, password-protected OneDrive folder and email you the link.
The password will be sent in a separate message for security.
Please allow up to 15 business days for processing, in accordance with California Health & Safety Code §123110.
If your request is time-sensitive, please include “URGENT – RECORD REQUEST” in your subject line.
How to Request Superbills

- Date of service/appointment
- *Picture, Screenshot, or PDF of your confirmation/receipt of payment from stripe for the records (*$15 per superbill)
